How to Report Fraudulent Check, ACH, and EFT Transactions

This article explains how to report unauthorized transactions involving checks, ACH withdrawals, electronic funds transfers (EFTs), or Digital Banking activity.

NOTE: This article does not apply to debit card fraud, credit card fraud, or merchant disputes.

What Types of Activity Does This Article Cover? 

Examples include:

  • Checks you did not write or authorize

  • Stolen or altered checks

  • Unauthorized ACH withdrawals

  • Unauthorized electronic funds transfers (EFTs)

  • Unauthorized Digital Banking transactions

  • Account activity initiated without your permission

How to Report Fraud

If you believe unauthorized activity has occurred on your account: 

Option 1: Visit a Branch

Visit any Peak Credit Union branch and complete the required fraud documentation.

Option 2: Call Us & Complete Fraud Documentation Through DocuSign

Call (800) 258-3115, and a team member will review your situation and provide instructions for completing fraud documentation electronically through DocuSign.

You may be asked to provide:

  • The date you first noticed the activity
  • The transaction date(s)
  • The transaction amount(s)
  • A description of the unauthorized activity
  • Any additional information that may help with the investigation

What Happens Next?

Once your documentation is received, it will be forwarded to Peak's Fraud Investigations Department for review.

During the investigation:

  • Additional information may be requested
  • Account activity may be reviewed
  • Fraud prevention measures may be recommended

When the investigation is complete, you'll receive a letter by mail explaining:

  • The results of the investigation
  • Any actions taken
  • Any additional information or next steps

Protect Yourself from Fraud

Please note, OUR TEAM WILL NEVER EVER ASK YOU TO GIVE THEM ANY LOGIN INFORMATION OR SECURE ACCESS CODE. Peak Credit Union never needs to have that information.

Peak Credit Union will never ask you for:

  • Your Digital Banking password
  • Your Login ID
  • A Secure Access Code
  • Multi-factor authentication verification codes

If anyone claiming to represent Peak asks for this information, do not provide it.

  • Report the activity as soon as possible by contacting Peak. Acting quickly allows the investigation process to begin sooner and may help prevent additional unauthorized transactions.

Will I need to complete paperwork?

  • Yes. Fraud investigations require documentation describing the unauthorized activity. This can typically be completed at a branch or through DocuSign.

What if I'm not sure whether the transaction is fraud?
